Functional Independent Measure (FIM)

The Functional Independent Measure (FIM™) is used in inpatient rehab settings to assess changes in functional status. It uses a 7-point ordinal scale to evaluate occupational performance for 18 items, measuring the severity of disability on the basis of care burden, that is, need for assistance from another person or assistive device, rather than impairment.

Fun fact: The IRF-PAI (Section GG) uses elements of the FIM™ (collectively referred to as the FIMTM).

Reliability and Validity:

  • Interrater ICC = 0.94, test-retest ICC = 0.93 (Chau et al., 1994)

  • The Motor FIM demonstrated Concurrent validity with the original 10-item Barthel Index and the 5-item short-form Barthel Index at admission with a Spearman’s r = 0.74 and ICC = 0.55. At discharge, the Spearman’s = increased to 0.92 and ICC = 0.86 (Hsueh et al., 2002).

Katz Index of Independence in Activities of Daily Living

The Katz Index of Independence in Activities of Daily Living measures a person’s independence in activities of daily living and monitors the effectiveness of treatment. There are 6 areas measured with a score of 1 for “Independent” (no supervision, direction, or personal assistance required) or 0 for “Dependent” (requiring supervision, direction, personal assistance, or total care).

Kettle Test

The Kettle Test assesses cognitive functional performance. The client is observed through the following tasks: Opening a water faucet, filling the kettle with 2 cups of water, turning off the faucet, assembling the kettle, attaching the electric cord to the kettle, plugging the electric cord into an outlet, turning on the kettle, assembling the ingredients, putting the ingredients into the cups, picking up the kettle when water boils, pouring the water into the cup, then adding milk. The maximum score is 52, with the highest score being indicative of severe impairment.

Montreal Cognitive Assessment (MoCA)

The Montreal Cognitive Assessment (MoCA) assesses multiple cognitive domains across 11 categories to measure cognitive impairment and dysfunction. A score of 26 or greater is considered “normal”, with 30 being the highest possible score.
